Support Initiatives Northwest Ltd Domiciliary Care Services enable the people we support to live an independent and dignified life within their own homes for as long as they are able  or wish to do so by providing supports and services that are tailored to their unique and individual needs, abilities, goals, aspirations and preferences. Services are flexible and responsive to changing needs.

Support Initiatives Northwest Ltd supports individuals aged 18 and over with moderate to severe levels of the following needs:

            •           learning disabilities

            •           challenging behaviour

            •           physical Impairments

            •           sensory disabilities

            •           mental health Impairments

            •           multiple special needs

            •           acquired brain injury (A.B.I)

            •           other complex and special needs

 

Involvement of the People we support, relatives, carers and advocates

Support Initiatives Northwest Ltd is committed to strong and meaningful involvement of the people that we support, relatives and advocates in the planning, provision and review of support services.  A core value of the support scheme is the right of the people that we support to be decision-makers in identifying the type of supports and services they receive.

Support Initiatives Northwest Ltd also involves the people that we support in the periodic review of the quality of support services it provides. This is accomplished through various means, including focused discussions on needs and support services, satisfaction surveys, quality of life assessments, reviews and person-centred planning.

 Individualised service planning

Services within the Support Initiatives Northwest Ltd bespoke support services are person-centred, user-led and individualised. Comprehensive assessments are conducted for each person that we support with a focus on their strengths, needs, aspirations, interests and preferences.  Self- determination is emphasised throughout the assessment process. Based upon the information developed during the assessment process, a personalised service plan is developed. With the active involvement of the person that we support, the type of supports, services and activities most appropriate to their current needs, abilities, preferences, goals and aspirations are chosen.  The main emphasis will be utilisation of mainstream community resources and settings.

Self-Directed Support (SDS)/Individual Budget

Support Initiatives Northwest Ltd can develop and deliver support solutions in conjunction with SDS. We can support people who require support services to be in control of their own support package, and to give them the choice & control in the provision of services that they require.
