Our supported living and community outreach services offer different models of support for an individual. Whether in someone’s own home, flat schemes or shared accommodation, we have a range of individual flexible packages available from 24 hours, to just a few hours of support per week within the community, depending on your needs.

We encourage and support you to live a full and varied life, including;

Keeping you safe and healthy
Developing your domestic and life skills e.g. shopping, cooking, cleaning etc.
Helping you to manage your money
Providing you with sound, trusted advice and advocacy
Being part of your community e.g. social groups, attending local events and access to other local services
Supporting your aspirations to learn new things e.g. to attend college, work placements, volunteering and paid work

Throughout all of our services we focus on personalised support, tailoring services to the needs and aspirations of each individual; supporting them to lead the lives they want as active citizens in their communities.

Our services are usually commissioned by NHS Trusts and Local Authorities and may be accessed through GP / Social Service referral.
