Southport & Ormskirk Hospital NHS Trust - Adults at Risk Team
The Adults at risk team is a liaison service for adults who have additional needs and may be at risk when using the Trust’s services.
The role of the team is to support patients and carers through the hospital system more smoothly. Often by making “‘reasonable adjustments” to care and where necessary facilitate referrals for other support.
As well as helping patients, the team also has a role to educate and support staff on how to meet the needs of people with additional care needs across the Trust, and the importance of referring patients to the team.
The team can also ensure that patients receive accessible information. This can be in the form of bespoke easy read information relating to their specific condition. The role of the team involves:
- Pre-admission planning
- Service user education
- Carer support
- Pre discharge planning
- Facilitation of out-patient appointments
- Advice on issues such as communication, risk management, and consent to treatment
