Sefton Community Learning Disability Team

About our service

We take a person centred approach to working with people who

• have a learning disability and complex support needs
• are registered with a Sefton GP
• are over the age of 18
• whose needs cannot be met by generic services

We provide support in a range of areas which include, but are not limited to; 

• assessing mental health needs and delivering interventions
• developing positive behavioural support (PBS) plans
• ensuring individuals' physical health needs are met
• working pro-actively to prevent and manage crisis
• participating in capacity and best interest decision making, if appropriate
• delivering a range of specialist assessments and interventions
• delivering individualised training programmes to support service users with additional complex needs

About our team

We are a multi-disciplinary team working across the Sefton area in a variety of settings, offering assessment and interventions from nurses, psychiatrists, physiotherapists, speech and language therapists and clinical psychologists, with assistance from our support workers and clerical staff. We work well together as a team, working jointly with service users, carers and other people/services when required. 

Working in a person centered way, we take a human rights based approach to keeping people safe and well and meeting individual needs, whether this is in a group or one to one environment. We pride ourselves on our core values of supporting and respecting others.
