Community Learning Disabilities Team (South Sefton-Health) - Mersey Care
A health service for adults (17.5 plus) who have a learning disability and additional complex health needs including:-
- Additional mental health needs.
- Challenging behaviour.
- Physical health needs, which require additional support into generic health services.
- Some forensic issues.
- Emotional or psychological trauma.The team consists of nurses, speech & language therapists, support workers and consultant psychiatrist and psychologists.
The team access appropriate generic health services whenever possible working as a client-centred approach.Close working partnerships have been established with social services, provider agencies (i.e. voluntary agencies), primary care centres, police and probation services.
We also have two other teams based at Hesketh Centre, Southport and also 29 Wright Street, Southport.
