Information (Hospital) Passport

The Information Passport can be used by anyone who needs to share information about themselves, but may have trouble doing so. This could be because they have difficulty communicating, or remembering information that they may be asked when in Health or other appointments. 

The Information Passport is owned and kept up to date by the person or people who provide care for them and taken and shared when they choose, or just choose to share certain sections with others when needed.

The Information Passport is recognised in Health services and can be adapted in many others.  Open the attached PDF and print off to use at health or other appointments.  It  has a title Hospital Passport as it was developed to help in Health services, but has been shown to work well in different situations too.
