The enhanced Pre-school pathway

The pre-school enhanced pathway is an integral part of the 0-19 Children’s teams delivering the healthy child programme 0-5 years.  The emphasis is on ensuring all Sefton children have the best start in life. It is child focused and supports parents/carers to provide the most positive environment for their child. Through early identification of needs and the provision of early support, aiming to improve health outcomes for children. The enhanced pathway is delivered by a health visitor who will offer additional visits starting in pregnancy, complementing the core healthy child programme offer which is offered universally to all families. The pathway is offered to first time parents’ age 19 and under and first time parents over 19 who have been identified by their midwife /other health professionals as benefiting from the enhanced pathway due to additional needs.

School aged Enhanced Service

The school aged Enhanced service is delivered by our school nurses with an emphasis on early intervention. We work with children and young people who are referred to one of Sefton’s pupil referral units which are Jigsaw and Impact including the Oakfield and Daleacre Campus. We also offer our service to children and young people who live in Sefton who are receiving elective home education, the gypsy/traveller population and identified young people aged 16-19 years who are not in education, employment, or training and have not attended a school or college in Sefton.

Our school nurses provide a link between health, education and the wider children’s and young people’s services providing guidance and support on a range of health related issues. An initial health needs assessment will be offered, this may be undertaken at the education provision or at home. We always aim to contact the parent or carer to include them in the assessment and we will liaise with the pupil referral unit for the young people who are assessed in the education provision. We will then agree an action plan together based on the outcome of assessment which may include completing a brief intervention, referral to other agencies or completing an early help assessment to enable a co-ordinated plan of support for the children, young people and their families.

The Enhanced Team provides a key link between health, education and the wider children’s and young people’s services providing guidance and support on a range of health related issues.

Referrals to the service are through the local authority, school health and health visitors and other professionals.

We will then agree an action plan together based on the outcome of assessment which may include completing a brief intervention, referral to other agencies or completing an early help assessment to enable a co-ordinated plan of support for the children, young people and their families.
